Sam drove the Impala from the bar to the motel (off-screen, though) in Bloodlust (2x03), and there's a possibility that there are other such instances (HotH, for example) but on-screen? No, not a single time far as I can remember. :)

Nope, that was a black jacket. Dean hasn't worn the leather jacket since Roadkill (and then it was just in a very brief flashback scene of the boys doing research in a library).

Maybe it was decided that that too much wardrobe continuity was a bad thing in a show about boys who are prone to getting hurt and bleeding all over their clothes?

Though if that were the case, they would have done better to retire the jacket after having Dean bleed on it on screen.
